Physiatry Specific EPA/OPA Project

The U.S. National Library of Medicine defines ‘clinical competency’ as the capability to perform acceptably those duties directly related to patient care. The Accreditation Council for Graduate Medical Education (ACGME) has further delineated clinical competency into six (6) core competencies of medical trainees. As part of the Next Accreditation System (NAS), specialties were asked to create milestones specific to their training programs. Milestones are defined as competency-based developmental outcomes that can be demonstrated progressively by residents and fellows from the beginning of their education to graduation to unsupervised practice of their specialties. These specialty-specific milestones were written as sub-competencies under the rubric of core competencies.

As specialties adopt milestones, there have been some challenges. In 2004, the concept of Entrustable Professional Activities (EPAs) was created. EPAs are observable units of professional practice that can provide a link between competency-based medical education and clinical practice. A subcommittee of the AAP Education Committee identified and validated the following set of EPAs to serve residency training programs in the specialty of Physical Medicine and Rehabilitation (PM&R).
