In a perfect world, providers could help people get
healthy and stay healthy by addressing all the conditions that affect overall
health. It’s called an “upstream” approach—one that takes into account the
social and environmental sources of health problems that make their impact
where we work, live and play. This session will outline the tools and
considerations case managers and health care organizations can use to implement
an upstreamist mindset.
Learning Outcomes:
- Define the upstream effect and how it is different from traditional models of care delivery;
- Describe at least two non-physical factors that affect patient health; and
- Apply the upstreamist approach to enhance case management practice.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
