Listen and Lead: The Case Manager as Expert Communicator

Description:
No two patients are alike, so creating a care plan that both improves patient health and helps them meet their personal goals is a nuanced art. But expert communication skills can be learned and leveraged for results—engendering trust, rapport and true patient engagement. This session will address the dual roles of listening and leading in case manager communication--with patients, caregivers, and other members of the care team—for optimal results.

Learning Outcomes:

  1. Explain the goals of communication for patient engagement; 
  2. Describe the role of communication in positive interaction with families, caregivers and other members of the care team; and 
  3. Discuss effective communication when interacting within and across systems that affect patient care and outcomes."

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
