The Affordable Care Act accelerated broader societal trends, pushing adoption of value-based payment models that elevate measurement and leverage technology for more efficient, effective care. With new rules come new roles: What role do case managers play in the race to measure, manage and migrate to more accountable care? This session will examine the sociological, economic and technology dynamics of a health system in transition, and the power of case managers to shift the tide toward patient-centered care.

Learning Outcomes:

1. Summarize the economic and technology realities that undergird current trends in health care;
2. Reflect on the cultural, ethical and sociological values that will drive the ascendancy of patient values and shared decision-making, and the role case managers play as patient advocates and educators; and
3. Discuss the relationship between the drive toward value-based care and the role case managers play in achieving patient-centered care and health care value.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
