Coordinating the Coordinators: Opportunities, Pitfalls and Barriers to Team Success

As the largest integrated health care system in the country, the VA was an early adopter of team-based care coordination and managing care transitions. This session will frankly highlight barriers to success and lessons learned along the way, as well as the evolution of the teams that now leverage the strengths of both nurse case managers and social work case managers. There is much to learn about identifying complex patients’ primary needs and working as a team across settings—and today, coordinating care for active duty military as they transition to ongoing, VA-based care.

Learning Outcomes:

  1. Summarize the evolution of PACT teams within the VA and the distinct roles of both social work and nurse case managers in the system;
  2. Describe common barriers to effective team dynamics and solutions to overcome those barriers; and
  3. Apply lessons learned from the VA experience to non-VA settings for improving care coordination across diverse settings and organizations using a team-based approach.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
