High-Value Care: Understanding High-Need, High-Cost Patients

Description:
The distance between acute care and a soft landing at home is becoming smaller. Under the Affordable Care Act, the CMS Innovation Center designated $500 million to enable community-based care transition programs. The funds are an investment in practices that better meet the needs of patients as they transition to community settings—needs that go far beyond getting to the next doctor’s appointment—and reduce unnecessary readmissions. This session will describe how organizations are focusing on patients with complex needs to achieve high-value care transitions.

Learning Outcomes:

  1. Discuss the characteristics of high-need, high-cost patients: Who are they?
  2. Connect the special needs of these patients to how they use health care; and
  3. Describe steps health care organizations can take to identify high-cost, high need patients and better meet their care transition needs through case management.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
