Data, analytics and the shift to value-based care

Description and CE information:
Savvy use of data and analytics holds the promise of supporting quality improvement, patient care delivery and care coordination across settings—all aspects of value-based care. This session will examine how technology is helping case managers and other members of the care coordination team identify patients at risk, reduce gaps in care, improve patient engagement and empower the patient to regain control.

Learning Outcomes:

  1. Summarize two examples of how technology is used to support care coordination and manage care transitions;
  2. Explain the role of data analytics for population health improvement; and
  3. Discuss the role of health IT for patient engagement and empowerment.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
