Description:
The VA health system is the largest integrated health system in the country. To serve more than 9 million enrolled veterans every year, the VA systematically works to identify, test, and evaluate new approaches to efficiently and effectively provide care. This panel will explore innovations in VA health care that involve case management, tapping into the expertise of panelists who represent nursing, social work and mental health case management.
Learning Outcomes:
- Describe the role of the multi-disciplinary case management team for care coordination across settings.
- Explain the value proposition of care integration--behavioral health, physical health and wrap around services-for a large health system like the VA; and
- Identify three emerging health care delivery innovations involving case managers that the VA is piloting which can be operationalized for other organizations
