Hurdle Training: Overcoming Barriers and problem solving

Description:
Short-term list-based siloed inpatient or outpatient, medical or behavioral health case management designed to target short-term isolated problems in general patient populations with diffuse but generally low-cost health challenges.

Longitudinal relationship-based integrated medical and behavioral health multidomain case management designed to reverse health and cost outcomes; measure clinical, functional, cost, satisfaction, and quality of life improvement, and return discharged patients with health complexity back to standard clinical care.

This hurdle training will discuss transition from existing segregated medical and behavioral case management for targeted health issues to a system in which appropriately trained medical and behavioral case managers provide integrated, outcome oriented, multidomain case management to targeted patients with high cost and high need.

Learning Outcomes:

  1. Provide data on the poor health outcomes and high patient costs associated with segregated medical and behavioral health case management
  2. Describe existing case management models currently used to “solve” (reverse) comorbid medical and behavioral health problems, especially in complex patients
  3. Discuss the development of cross-disciplinary, relationship-based, integrated medical and behavioral case management programs in overcoming barriers to total health and function
  4. Summarize a systematic method for documenting barrier reversal and improved health and function in patients participating in value-based integrated case management programs

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
