This webinar will focus on leveraging value based payment and digital health tools to improve efficiency and effectiveness of care.

Learning Outcomes:

  1. Create awareness of the current and future landscape influencing the patient, family and care provider(s) across the care continuum in the evolving value based payment landscape.
  2. Understand how the evolving digital health landscape can better support the health of individuals and communities.
  3. Enhance understanding of lessons learned in disasters like Hurricane Katrina that apply to care models every day.
