This webinar will be an interactive discussion using case studies, visual graphics and demonstration to help case managers gain confidence in their abilities to help their clients manage their social needs. This session will address why it is important to ask those uncomfortable questions to get to the true needs clients face, how case managers can help their clients maintain their dignity by empowering them to access resources on their own. We will also delve into the partnerships that are working in organizations across the country, and how collecting and understanding data can help case managers and their organizations make better decisions for healthier outcomes.
Learning Outcomes:
- The Learner will be familiarized with new data sources to better understand and assess the health of their communities on a macro level.
- The Learner will be equipped with the resources on a micro level to connect with and intervene earlier with clients in need, resulting in better overall health outcomes for those clients.
- The Learner will be better prepared to recognize community trends, allowing him/her to identify and implement effective interventions.
