Description

Patrick Gordon, MPA, is the Chief Executive Officer for the Rocky Mountain Health Plans (RMHP) based in Colorado. He has years of experience implementing a wide array of practice transformation, payment reform, and technology innovations, including the statutory Medicaid Payment Reform for Medicaid Enrollees (PRIME) initiative. During this session, Patrick will discuss the importance of integrated team creation. He will identify the roles and skills most valuable in an Integrated Care Coordination Team (ICCT) and how community resources can be included for maximum effectiveness.

    Patrick Gordon is the Chief Executive Officer for the Rocky Mountain Health Plans (RMHP) based in Colorado. UnitedHealthcare is a division of UnitedHealth Group (NYSE: UNH) which is a diversified health and well-being company with a mission to help people live healthier lives and help make the health system work better for everyone. Patrick previously served as the Vice President of the Rocky Mountain Health Plans, where he led the implementation of a wide array of practice transformation, payment reform, and technology innovations, including the statutory Medicaid Payment Reform for Medicaid Enrollees (PRIME) initiative. Patrick also procured and led coalition development for the social risk factor screening in clinical sites, community navigation for patients with complex needs and social information exchange. Additionally, Patrick led the design and implementation of EasyCare Colorado. Since working within a nationally recognized community health plan, Patrick has created and executed RMHP’s successful global budget and payment reform program for Colorado Medicaid. He served as Executive Director for the Colorado Beacon Consortium, after leading the acquisition of Cooperative Agreement from the Office of the National Coordinator for Health Information Technology (ONC). He designed and implemented SHAPE, an integrated behavioral health and primary care payment reform sustainability project, in partnership with a research team at the University of Colorado Department of Family Medicine. Patrick earned a Bachelor of History, Philosophy from Metropolitan State College of Denver and a Masters of Health Policy & Economics from the University of Colorado. He served on the Executive Committee for Reunion Health and was a committee member on the 2017 PCMH Standards Advisory, National Committee for Quality Assurance (NCQA).
