Patrick Gordon, MPA, is the Chief Executive Officer for the Rocky Mountain Health Plans (RMHP) based in Colorado. He has years of experience implementing a wide array of practice transformation, payment reform, and technology innovations, including the statutory Medicaid Payment Reform for Medicaid Enrollees (PRIME) initiative. During this session, Patrick will discuss the importance of integrated team creation. He will identify the roles and skills most valuable in an Integrated Care Coordination Team (ICCT) and how community resources can be included for maximum effectiveness.

  • Identify the skills and roles valuable in an Integrated Care Coordination Team (ICCT)
  • Discuss how other community resources (Social Determinants of Health-SDoH) can be integrated for effectiveness
  • Describe the payers interest in effective ICCTs
