Help Me! Transitioning From Pediatric to Adult Care

Each year thousands of young adults with and without special health care needs age out of pediatric care. Pediatric and adult clinicians often do not communicate with each other in this transition process and often do not improve their practice processes to improve HCT planning, transfer and integration into adult health care, the three pillars to improving transition services for youth and families. Assuring effective transitions across the health care continuum is an important standard of practice. This session will describe a structured process that will make improvements possible for the transition.

Learning Outcomes:

  • Describe current drivers urging both peditric and adult health care professionals to recognize, prepare, and provide transition-to-adulthood services to adolescents and young adults.
  • Employ the structured approach to HCT accessing free and publicly available resources and tools.
  • Identify care management interventions to increase client engagement and prevent discontinuation of care during transition from pediatric to adult care.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
