What's Next: Rethinking the Team

There is a need to find simplicity in the chaos of health care delivery, to rethink the role of the case manager as advocate to coordinate an interprofessional team to best provide the right care, at the right time, in the right setting, at the right cost, with the right outcomes for the patients and clients served. In this session the presenter will describe how case managers can, with curiosity, perseverance and good old-fashioned grit, use the acknowledgement of what we do not know to create a concept of team, based on interprofessional care – a partnership between a team of health providers and the patient in a participatory collaborative and coordinated approach to shared decision making around health and social issues.

Learning Outcomes:

  • Examine pre-pandemic and current mandates of the Affordable Care Act. 
  • Identify the impact of COVID-19 on the future of case management practice in value-based care. 
  • Explain the concept of the interprofessional team and its impact on the patient and case manager.
  •  Apply knowledge gained to rethink the role of the case manager in the formation of an interprofessional team.

The Commission for Case Manager Certification (CCMC) is accredited as a provider of nursing continuing professional development by the American Nurses Credentialing Center's Commission on Accreditation.
