  • John Umhau, MD, MPH, CPE, FASAM, CDR

    Dr. Umhau is a practicing addiction medicine specialist and founder of Alcohol Recovery Medicine, an online private practice. He was a founding member of the CCHF Board of Directors and past president of the Academy of Medicine of Washington D.C. and a lifelong member of CMDA. Dr. Umhau received his B.S. Degree from Davidson College, an MD from Wake Forest University, and an MPH from Johns Hopkins University School of Public Health where he completed a residency in Clinical Preventive Medicine. As a Commander in the USPHS, Dr. Umhau served as a Senior Clinical Investigator at the National Institutes on Alcohol Abuse and Alcoholism for over 20 years and has published more than 40 peer reviewed papers on alcoholism and nutritional neuroscience.

  • Warren Yamashita, MD, MPH

    Dr. Yamashita is a fourth-generation Japanese America whose great grandparents and grandparents were interned in WWII prison camps according to America's Executive Order 9066. Yet, what the enemy meant for evil, God meant for good. Dr. Yamashita became the first in his family to trust Jesus as Lord and Savior, followed by his brother and fathers. Dr. Yamashita is a Family Medicine physician who is currently an Addiction Medicine Fellow at Stanford University. He serves on CMDA's R2ED (Racism, Reconciliation, Equality and Diversity) Committee, where he is excited to see Jesus breaking the chains of spiritual oppression one heart, relationship and community at a time. Dr. Yamashita has been a CMDA member since April 2015.

  • Michelle Knapp, DNP, PMHNP-BC

    Michelle Knapp, DNP, PMHNP-BC is the founder of Wholistic Psychiatry, PLLC, in New York, New York and Venice, Florida. She is a Fellow of the International Society of Nurses on Addictions and an appointed committee member of the American Psychiatric Nurses Associations Addictions Steering Committee. Dr. Knapp received her BSN from Cleveland State University, Master’s Degree from New York University and her Doctoral degree from Chamberlain University. Dr Knapp helped to develop the first PMHNP residency training program in addictions at UCSF and the San Francisco VA in 2016. She subsequently served as the director of the Psychiatric Nurse Practitioner Program and Substance Use Specialty Sequence at New York University where she was a primary investigator with the NYU Langone NIDA-funded SARET (Substance Use Research Education and Training) Program. She also received SAMHSA grant funding funneled through NY State to develop clinician training for treating women who use substances. She is the author of several peer-reviewed articles and textbook chapters on the topic of substance use across the lifespan. She has also presented at more than 20 international, national, and regional conferences on relevant topics such as substance use and prescribing buprenorphine.

  • Sara Rahkola, MD, FACP

    Sarah Rahkola, MD, FACP, is a Clinic Medical Director for Primary Care Internal Medicine at Providence Medical Group, Newberg, Oregon. She is a fellow of the American College of Physicians, serving as finance chair of the Oregon Chapter, as well as being a member of CMDA where she serves on the WPDC commission. After earning a BA in Physics, she completed her MD degree at Oregon Health & Science University, followed by residency and chief residency at Providence St. Vincent Medical Center in Portland, OR. Due to the high incidence of obesity in her practice, she has recently earned board certification in obesity medicine, and enjoys teaching other physicians how to increase their competency in treating this disease.

  • Kurt Bravata, MD

    Kurt Bravata, MD is a Family Physician specializing in primary care, geriatrics, and addiction medicine at Citizens Memorial Healthcare. Dr. Bravata received his medical degree from Ross University and clinical training at Bronx Lebanon Hospital in Bronx, New York in 2013. He became a Fellow of the American Society of Addiction Medicine in 2019 and serves as the secretary of ASAM Midwest Chapter. He is a founding member and current Vice-chair of the CMDA Addiction Medicine Section.

    Dr. Bravata is co-founder and medical director of the integrated outpatient addiction treatment program based at the Butterfield Park Medical Center in Bolivar, MO, with other satellite outreaches within the Citizens Memorial Healthcare system. The CMH Addiction Treatment Program utilizes the latest pharmaceutical interventions in combination with proven behavioral health therapies, peer support, and case management. Dr. Bravata is DATA 2000 waivered and board certified in both Family Medicine and Addiction Medicine. He is a member of the Recovery Coalition of the Ozarks, the Missouri Coalition of Recovery Support Providers, AAFP, MAFP, ASAM, and the Dallas County Live-Well Alliance of which he is a founding member. Dr. Bravata recently served on the MAFP Board of Directors and is involved in advocacy on Family Medicine and Addiction Recovery issues.
