** This session earns SW, RN, and CCM ** In order to assure the best possible outcome for a patient with a catastrophic illness or injury, it is necessary to understand how various types of care and professionals contribute to the final outcome. In this presentation, including both PowerPoint and video illustration, participants will learn about the various types of rehabilitation care including acute rehabilitation, rehabilitation in a long term acute care hospital, skilled care, residential care, team based comprehensive outpatient rehabilitation and outpatient single discipline services. A case example will be followed that participated in multiple levels of care to show how the focus changed throughout the rehabilitation process and how it impacted the final outcome. Information will be provided on various rehabilitation technologies and specialty services that can be incorporated into the plan of care. Particular emphasis will be placed on discussing the needs of brain injury patients from those still in a coma through return to work as well as persons with spinal cord injury and pulmonary needs. Finally, long-term outcomes of persons who require inpatient rehabilitation will be presented with discussion of what types of follow-up might be important to address their needs. OBJECTIVES:
