The Other Vital Signs: Reimagining Healthcare Through Social Drivers of Health

Health outcomes are influenced by far more than what happens in a clinic or hospital. Where people live, how they access food and transportation, whether they feel safe and connected, and even whether they can access digital resources can shape their ability to manage their health and navigate the healthcare system. These factors are not simply background circumstances—they are critical drivers of outcomes, equity, and whole-person care.

This presentation reframes social drivers of health (SDOH) as “the other vital signs” and challenges participants to look beyond identifying and documenting social needs toward creating meaningful, upstream solutions. Through real-world examples, interactive discussion, and case application, participants will explore how community partnerships, technology, measurement, and expanded care teams can transform how social needs are addressed across the care continuum.

Attendees will leave with practical strategies for integrating SDOH into care delivery, strengthening connections between healthcare and community resources, and measuring outcomes that reflect what matters in patients’ everyday lives. The session will highlight how care teams—including case managers—can play a pivotal role in moving from screening for social needs to changing the conditions that influence health.

  1. Explain why social drivers of health are central to outcomes, equity and whole-person care.
  2. Differentiate between documenting social needs and designing upstream solutions.
  3. Identify practical strategies to integrate partnerships, measurement and care team expansion into care delivery.
