From Volume to Value: Where Value-Based Care Is Going and What It Means for Case Managers

No Ratings

The transition from fee-for-service to value-based care continues to reshape how healthcare is delivered, coordinated, and financed. Accountable care organizations and other value-based models increasingly place responsibility on providers not only for individual services, but for the quality, cost, and coordination of care across settings.

This session will examine where value-based care stands today, the direction federal policy is taking, and what these changes mean for case managers. Using current Medicare accountable care initiatives and emerging models as examples, the presentation will explore the growing emphasis on care coordination, transitions of care, beneficiary engagement, quality, data sharing, and management of medically complex patients. The session will connect these policy changes to the practical role of case managers and examine why their work becomes increasingly important as healthcare organizations assume greater accountability for patient outcomes across the continuum of care.

  1. Describe the evolution of Medicare value-based care and key trends shaping the continued movement from fee-for-service toward accountable care.
  2. Identify how value-based care changes incentives and responsibilities related to care coordination, transitions of care, quality, patient engagement, and management of medically complex patients.
  3. Explain the role case managers can play in helping healthcare organizations succeed under value-based care by improving coordination across settings, addressing gaps in care, and supporting better patient outcomes.
