Redesigning Care for Bundled Payment Success

This session will be discussing the BPCI-A program and detailing best practices across the continuum that have supported proven success. The Bundled Payments for Care Innovation Advanced (BPCI-A) was started in 2018 as a continued development from the original BPCI program. BPCI-A is an Advanced Alternate Payment Model (APM) in which healthcare providers agree to receive a single payment for physician, hospital, and other healthcare services for specific diagnoses within an episode of care. An episode of care is defined as an anchor stay and 90 days following the anchor stay. During this time, the healthcare provider is financially accountable for the bundle. The CMS Innovation Center created the BPCI-A model to encourage development of innovative solutions for care redesign, care coordination and practice transformation, with the intent of improving quality and lowering the cost of care. It is critical to the success of a bundled payment participant to gain control of the continuum of care for a 90 days period and to redesign care for the best quality outcomes for the patients. Our efforts in redesigning care for our patient population has proven results in decreased readmission, more appropriate skilled nursing and acute rehabilitation placement and enhanced communication with home health providers. At our largest acute hospital with most BPCI-A volumes we showed that our cardiac procedures 90-day readmissions have a target of 11.4% our baseline performance was 18.4% last claims data showed (Q3-2020) 9.7%. Cardiac Care in general showed a readmission target of 23.6% and a baseline of 27.6% with an improvement to 23.0%. Medical and Critical Care readmissions showed a target of 21% and baseline of 26.3% with our recent numbers at 22.5%. The session will be of interest for acute and post-acute providers caring for BPCI-A patients across the continuum and in the community. The session will cover best practices in handing over care, preventing readmissions, collaborative communication; etc. If your organization touches the BPCI-A patients then this session is a must.
