Respecting Choices: A Guide to Advanced Care Planning

As we are all aware there are many challenges in health care today. One issue that continues to concern many health care providers is the need to understand the patient’s goals of care and how to meet those goals. Case managers are often in the position to assist in meeting the needs of the patient and are the bridge between the healthcare provider and the patient and their families. This presentation will help the attendees to develop a toolkit for advanced care planning with patients and their families.
The ability to clarify for patients, as well as health care providers, the difference between palliative care and hospice care will be an aid in the advanced care planning process. As will be presented, patients often have different goals for care but are not able to attain those goals. This presentation is focused on more patients meeting their goals for their care.

Objectives:
At the end of this presentation the participant will be able to:
1. Define advanced care planning and the impact on patients and their loved ones.
2. Develop a tool kit to assist patients, families and providers with choices of care.
3. Describe the difference between Palliative Care and Hospice Care.
