Session Description 
This symposium presents the findings from a cluster-randomized, stepped-wedge clinical trial in 16 dental clinics. The trial evaluated the effectiveness of a deliberative loop intervention in increasing providers’ adherence to an evidence-based clinical practice guideline based on the ADA’s pit-and-fissure dental sealant guideline. Previously in these clinics, we found sealants were the least frequently adopted approach to managing non-cavitated occlusal carious lesions (NCCLs). We also found that the organization had no clearly articulated strategies for the implementation of this guideline but instead relied on passive dissemination, continuing education, and clinical champions. As framed by the COM-B model of behavior change, this lack of infrastructure is a barrier of capability. According to Deliberative Democracy theory, the collective wisdom of those who would be affected by a policy can be engaged and expanded to identify solutions to policy problems. We engaged clinic staff in a three-step deliberative loop process providing clinic staff with background materials, facilitating small group discussions, and presenting an opportunity to share their informed opinions with clinic leadership. We hypothesized that the deliberative loop intervention would address the barrier of capability, leading providers to place sealants on more eligible NCCLs following their participation in the intervention compared with prior to their participation. Furthermore, we hypothesized that having an informed opinion and voice would mediate the relationship between experiencing the intervention and placing sealants. The intervention did not improve the rate of sealant placement, stakeholders’ perceptions of voice, or opinion. Future directions will be discussed.
