Description

The life care planning process specifies the longer term medical, psychological, and rehabilitative needs of the individual throughout their life time. This session discusses an assessment model to assess the individual’s ability to manage health information and technology. This data and technology readiness should help support the appropriate resources required.

Learning Objectives

  1. Understand the principles of the Data Management Capability Model (DCAM).
  2. Apply DCAM principals to patient data readiness assessments
  3. Understand how to include DCAM assessments within a life care plan as supporting foundation for case management functions and/or technology aides.
