Unique Considerations in Pediatric Life Care Planning

description

The development of a pediatric life care plan offers the practitioner challenges and rewards, and this Webinar will address the unique role of the life care planner in the creation of an individualized, comprehensive, and life-long care plan for a child with chronic health conditions. It will also examine how the life care plan establishes an integrated continuum that can result in improved outcomes, a reduction in complications, and appropriate use of resources for children with chronic illnesses and catastrophic injuries. It is hoped that this Webinar will serve as a resource for life care planning specialists to educate families about the expected goods and services that they are likely to encounter to provide optimal care for their children with special healthcare needs.
