Neuro Community Care will be addressing ways to increase a person’s (and their family's) quality of life who has a chronic injury or illness and ways to support the person as a whole.

Learning objectives

  1. Define ways in which practitioners can support individuals with chronic injuries/illnesses.
  2. Recognize practitioner processes in assessment and case management.

  5. Compare medical case management with NCC’s case management model
  6. Demonstrate an understanding of social determinants of health and how it supports medical case management.
