Neuro Community Care will be addressing ways to increase a person’s (and their family's) quality of life who has a chronic injury or illness and ways to support the person as a whole.

Learning objectives

1. Define ways in which practitioners can support individuals with chronic injuries/illnesses.
2. Recognize practitioner processes in assessment and case management.
3. Demonstrate how practitioner programs and services impact clients.
4. Define how community support specialist (CSS) involvement enhances a client’s quality of life and health outcomes.
5. Compare medical case management with NCC’s case management model
6. Demonstrate an understanding of social determinants of health and how it supports medical case management.
