Working Upstream: Addressing the Social Determinants of Health in Rural Communities

This webinar is hosted by NACCHO and five recipients of the Achieving SDOH Population Improvements in Rural Environments (ASPIRE) award to discuss strategies for addressing the social determinants of health (SDOH) in rural communities. As public health approaches move more upstream, its essential for health departments to build solid community foundation to support the work. With funding from the Centers for Disease Control and Prevention (CDC), NACCHO identified five rural communities currently engaged in addressing the social determinants of health through planning and partnerships. The webinar will share the results of qualitative interviews with the sites that explore significant themes in upstream work. Each site will also share their own work as well as insights on key needs to successfully address SDOH in their communities.
