Mobilizing for Action through Planning and Partnerships (MAPP): Organize for Success/Partnership Development and Four Assessments Phases

Mobilizing for Action through Planning and Partnerships (MAPP) is a community-driven strategic planning process for improving community health. This module explores the first and third phases of MAPP which involve organizing the planning process, developing the planning partnership, and the Four MAPP Assessments: 1) Community Themes & Strengths Assessment; 2) Local Public Health Systems Assessment; 3) Community Health Status Assessment; and 4) Forces of Change Assessment.

Learning objectives:
By the end of this eLearning module, participants will be able to:

  1. Describe Phase One: Organize for Success/Partnership Development
  2. Describe Phase Three: Four MAPP Assessments
  3. List the four MAPP assessments
