Northeastern  WI AHEC is working with Game Ground Health and the Greater Wisconsin Agency on Aging Resources to conduct community base experiences providing patient assessments virtually to establish any social determinants of health preventing a patient from health care adherence. Student will complete this series of modules before they begin their patient assessments.
