COURSE DESCRIPTION

Dr. Christina Suh will introduce the Patient Activation Measure® (PAM®) survey, a tool for measuring patients’ activation, and discuss the research demonstrating the benefits of improving activation scores for patients with chronic kidney disease. She will highlight the role of the PAM Performance Measure (PAM-PM) in improving performance on quality measures and reducing measure burden for physicians.

Dr. Suh will be joined by Robert Taylor, MD, co-founder and Senior Medical Director of REACH Kidney Care, who will describe his experience using the PAM and how it has brought value to his practice and its patients.

During this webinar you will:

•Learn how you can use PAM to improve outcomes for your patients.
•Understand how measuring patient activation can help you more effectively target resources.
