Texas experienced an extreme COVID-19 surge, becoming a hotspot for months. The rapid onset and spread of COVID-19 in the presenters' community required physicians and staff to innovate quickly to address the many new challenges. Looking for solutions, the presenters developed and implemented collaborative and novel partnerships between generalists and palliative care specialists to help ensure that their core humanizing values were not lost in the pandemic. This presentation will review their successes and lessons learned with the intent of disseminating programs that are transferable to other institutions for implementation. These programs provided actionable examples of how to streamline communication efforts with families, incorporate teaching of serious illness communication skills to learners using the VitalTalk framework, and promote upstream discussions regarding advance care planning in serious illness for vulnerable, at risk patient populations.
