This VA Special Series session, Complex Discharge Planning for Veterans: An Interdisciplinary and Framework-Drive Approach, was originally presentated at the SGIM22 annual meeting in Orlando (April 2022).
Care coordination is a core competency for generalists. However, as patients' issues become more complex, there is an increasing need for frameworks and approaches to specifically identify what the problems are, what should be focused on, and who locally can help. This presentation illustrates how meetings and frameworks function through a case of a Veteran with cognitive and mobility impairment who struggles with poverty and social isolation.
