Thank you for attending the 2022 SGIM Mountain West Regional Meeting on November 4!   We value your input!  Please take a few minutes to complete the meeting evaluation. 

At the end of the evaluation you will be asked if you wish to request CME.  If so, you will be asked to identify how many hours you wish to request. The maximum number of CME hours available for this event is 5.75 hours. 
**Poster and networking sessions are not eligible for credit**

December 4, 2022 is the Deadline to complete the evaluation and request CME MOC. 

If you wish to claim MOC, you must complete BOTH the overall evaluation and the MOC Assessment. 
**If you request MOC, please be sure to populate your ABIM# and Day/Month (DD/MM) of birth. ** 

To claim your certificate: Once you have completed the Evaluation (and the optional MOC Assessment) you MUST click on the Certificate item to CLAIM, view and download your 2022 SGIM Mountain Wes Regional Meeting Certificate.  We recommend saving a copy for your records.
