Optimizing Healthcare for LGBTG Individuals in Case Management

Case managers work with diverse populations, including LGBTQ individuals. In order to be effective, case managers need to understand the unique needs of LGBTQ individuals. This includes understanding the health disparities that disproportionately impact LGBTQ individuals, such as inadequate preventive services, lack of adherence to clinical guidelines for chronic conditions, and challenges in finding providers. This presentation will provide information on key terms and concepts, clinical competencies in LGBTQ providers, and navigating the healthcare system. The presentation will also address the physical health, behavioral health, and social determinants of health.

OBJECTIVES:
  1. Recognize key terms and concepts with LGBTQ individuals
  2. Identify key issues with the healthcare system and providers for LGBTQ individuals
  3. Discuss how to coordinate care to meet the needs for the physical health, behavioral health, and social determinants of health
