Description
This session will serve as an introduction to
transition planning (often referred to as the “transition plan”) within an
IEP. Content will focus upon the three
areas in which post-secondary transition outcomes must be developed and the
ways in which age-appropriate assessments are utilized to develop both
post-secondary outcomes and annual IEP goals.
Levels of assessment will be discussed.

Upon completion of the webinar, participants will be able to:
- Accurately utilize transition language including: transition planning, age-appropriate assessment and post-secondary outcomes.
- State the three areas in which post-secondary outcomes must be developed for students in transition.
- Define the term “age appropriate assessment”.
- Identify at least one assessment that can be utilized to assist in determination each of the three post-secondary outcomes.
