Skills-Based Approach to Managing Chronic Health Conditions

These materials were originally developed for a seminar at the Healthy Schools Academy hosted by CDC funded national organizations* in February 2019.  This was a 2.5 day training-of-trainers event to support professional development activities in 17 CDC-funded Healthy Schools Grantee states.**

* CDC funded national organizations are the Alliance for a Healthier Generation, SHAPE America, Springboard to Active Schools, and the National Association of School Nurses (NASN).

** Grantee states are charged with supporting student health and academic achievement through physical education, physical activity, nutrition, and managing chronic health conditions – and the national organizations are charged with supporting the states.

NASN’s seminar at the Healthy School Academy was titled Skills-Based Approach to Managing Chronic Health Conditions.  It focused on foundational skills for managing chronic health conditions.  The seminar role modeled for trainers how to present the content and intentionally applied adult learning principles. 

The materials provided to trainers at the seminar are available here.  They were developed to be replicated for delivery in one- to three-hour segments, tailoring the segments to meet the needs of learners.
