Using post-discharge follow-up calls to coordinate care across settings is an important element within an overall strategy to reduce readmissions and improve patient outcomes. This presentation provides a case study overview of how a centralized, system of post-discharge phone calls can improve efficiency, decreasing readmissions and emergency department visits. It includes metrics to quantify the value-add of efficient post-discharge follow-up to enhance patient experience measures and demonstrate case management value.

  2. Discuss potential challenges and solutions associated with using patient experience measures to demonstrate case management value
