Social factors such as food and housing security, transportation and personal safety contribute to poor health outcomes. Case managers are the connectors for clients whose health depends on ensuring these all-too-human needs are met. In this webinar, the speaker will share strategies for establishing connections to community resources and developing relationships with these important partners.
Learning Outcomes:
- Describe effective strategies for implementing social determinants of health screening in primary care.
- Identify ways to leverage technology and data to better address patient's identified social determinants of health.
- Evaluate current community partnerships and identify ways to expand these key relationships and grow new partnerships.
