Implementing a Food Insecurity Screening Program

This session looks at food insecurity and its impact on health outcomes. Using a screening protocol to identify persons at risk of food insecurity, case managers can then link clients to food resources incorporating a follow-up plan into process.

Learning Outcomes:

  • Identify food insecurity using a screening protocol.
  • Explain the impact of food insecurity on health outcomes.
  • Describe the importance of linking members to food resources and incorporating a follow-up plan into the process.
  • Apply presentation content to inform social determinants of health interventions in other settings.
