Advance Care Planning in Primary Care

While studies show that the majority of people prefer not to die in a hospital, many of them do. Advance care planning conversation in primary care proactively addresses this issue and can reduce the use of acute hospital care at the end of life. This session will discuss processes that are being implemented in a primary care setting and preliminary outcomes of this intervention to date.

  • Explain how advance care planning may be implemented in primary care
