Hospital at Home: For Now or Forever?

speaker:

Jeannie LeDoux, RN, BSN, MBA, CCM, CPHQ, CTT+

Description:

The Hospital at Home (HAH) concept is not new but came to prominence during the pandemic when patients requiring a hospital level of care were either reluctant or unable to be admitted. CMS approved the expanded use of this model as a solution to the problem. In this session, the presenter will discuss the background of HAH, what requirements are necessary to provide this level of care safely, and what diagnoses are appropriate for HAH. In reviewing both the benefits and the cautions revolving around HAH, the presenter will discuss what the evidence shows and whether this is a limited option or a trend for the future.

Learning Outcomes:

  • Define Hospital at Home
  • Recognize diagnoses that are appropriate for this level of care
  • Describe the role of the professional case manager in this setting
