Description:

Bon Secours Mercy Health (BSMH) Care Management provides risk stratified end-to-end care management solutions to patients across the continuum of care. We have demonstrated improved performance across multiple value-based contracts in spite of COVID-19 and the challenging economic effects on communities and healthcare organizations. BSMH uses an interdisciplinary team-based approach to meet patients where they are to develop a patient-centered plan of care and work towards individual wellness. BSMH Care Management closely monitors outcome metrics to identify care management performance at a population and care manager level. This combination of outcomes and productivity data supports the autonomy of the care management team while providing identification of performance opportunities at an individual, market and system level.

This presentation will take the learner through the journey of BSMH care management transformation and discuss programmatic and strategic elements that helped the organization achieve the triple aim.
