Thank you for attending this MI-AIMH Training. There are required follow-up steps in order to receive your certificate. NOTE you must be signed in MI-AIMH RELATE in order to access and complete these steps:

1. Please complete your training survey - this can be accessed by scrolling down to the bottom of this page and clicking on the survey. You can also use this URL:

2. Once your survey is done, you can complete your training assignment. This step requires that you add your full name and organization tot he document. Once you have submitted your assignment, MI-AIMH staff will check and approve your attendance, payment and release your certificate.

**If your assignment says " Waiting on Instructor" you are all set and waiting on MI-AIMH staff!

3. Once your certificate is available, you will receive an email with a link to access your certificates. Note that your certificate are always available in your User Account/Portal
