** This session earns SW, RN and CCM ** Many case managers may be familiar with the phrase value-based care, but many not be aware how non-medical homecare is being leveraged across the country to serve as the most favorable and lowest cost solution to help our population age in place and receive the care that they need. In 2018, the Centers for Medicare and Medicaid Services granted permission to health plans to include non-medical in-home care as a supplemental benefit in their 2019 Plan year, and this presentation will review initial data, success stories, and challenges with the first year's implementation. The course will also share specific examples of how to engage with non-medical home care organizations, as well the important differences and risks between web-based, consumer directed providers vs. licensed homecare organizations.
